or telephone and we will contact you. Please provide us with
a telephone number and the best time to reach you if you have
to leave a message. An in-depth telephone interview is conducted
to see if our program will be a good match for you. If that is
determined then an application will be mailed and a suitable
"next available" date established.
The application, contract,
and privacy statement as well as the location of facility
where the workshop is conducted will be sent. The documents
must be completed and returned with prepayment 7 days
prior
to the scheduled workshop date. A pre-paid envelope is provided.
